“The Golden Ticket $100,000” Sweepstakes
In the event the Sweepstakes is, in the opinion of the Sponsor and/or Participating Promoter(s), compromised by viruses, bugs, non-authorized human intervention or other causes beyond the control of Sponsor and/or Presenting Promoter which corrupt or impair its administration, security, fairness or proper play of the Sweepstakes, Sponsor and/or Participating Promoter(s) reserves the right in its sole discretion to suspend, modify or terminate the Sweepstakes from point of impairment, and select the winners from among all eligible entries received prior to termination. Sponsor and its parent company, affiliates, subsidiaries, agents and advisers and the employees, officers, directors, shareholders, agents, and representatives of each are not responsible for any error, omission, interruption, deletion, defect, delay in operation of transmission, communications line failure, theft or destruction or unauthorized access to, or alteration of, entries. Sponsor and its parent company, affiliates, subsidiaries, agents and advisers and the employees, officers, directors, shareholders, agents, and representatives of each are not responsible for any problems or technical malfunction of any telephone network or lines, computer on-line systems, servers or providers, computer equipment, software or failure of email on account of technical problems or traffic congestion on the Internet or any web-site or combination thereof, or for any injury or damage to participant’s or to any other person’s computer related to or resulting from participating or downloading materials in this Sweepstakes.
Sponsor may prohibit an entrant from participating in the Sweepstakes or winning a prize if, in its sole discretion, it determines that said entrant is attempting to undermine the legitimate operation of the Sweepstakes by cheating, hacking, deception, or other unfair playing practices (including the use of automated quick entry programs) or intending to annoy, abuse, threaten or harass any other entrants or Sponsor’s representatives.
LEGAL WARNING: Any attempt to deliberately damage any on-line entry or web site or to undermine the legitimate operations of the Sweepstakes is a violation of criminal & civil laws. Should such an attempt be made, Sponsor and/or Participating Promoter(s) reserves the right to disqualify such entrant & seek damages from any such person to the fullest extent of the law.

The Joint Fundraising Campaign “The Golden Ticket $100,000” (the “Sweepstakes”) begins January 25, 2013 at 12:01 a.m., Eastern Time (“ET”) and ends July 10, 2013 at 11:59 p.m. ET (“Sweepstakes Entry Period”). Unless otherwise specified in this offer, online/telephone contribution herein referred to as (“Donation”) must be received by July 10, 2013 and mail entries must be received by July 17, 2013. This Sweepstakes may be presented in different creative presentations by different non-for-profit institutions (“Participating Promoters”). Sweepstakes is sponsored by United Lubavitcher Yeshiva of Ocean Parkway, 841-853 Ocean Parkway, Brooklyn, NY, 11230.
HOW TO ENTER: The Golden Ticket $100,000 Sweepstakes consists of five (5) separate entry levels: (1)-First prize level ($100,000 cash), (1)-Second prize level ($10,000 cash or trip), (1)-Third prize level ($5,000 cash or trip), (5)-Fourth prizes level ($1,000 cash prize per winner) 10-Fifth Prizes level ($500 Gift Card per winner); each with its own entry pool, prizes and drawing (each a ‘Drawing”). There are two (2) ways to enter each of the five Drawings:
(a) Donation Option:
(i) $100 - $179 donation will receive (1) entry ticket each in all prize Drawing.
(ii) $180 - $249 donation will receive (2) entry tickets each in all prize Drawings.
(iii) $250 - $449 donation will receive (3) entry tickets each in all prize Drawings.
(iv) $500 - $999 donation will receive (7) entry tickets each in all prize Drawings.
(v) $1,000 – 1,799 donation will receive (15) entry tickets each in all prize Drawings.
(vi) $1,800 – or up donation will receive (30) entry tickets each in all prize Drawings,
All Tickets must be submitted between 1/25/13 and 7/10/13 according to the instructions that you receive. Indicate your choice of the donation options set forth above and your choice of payment method. Telephone/online donations will be automatically entered in the drawings. Donations made at a live event will receive an entry ticket to be entered in the drawings.
(b) To Enter Without Making a Donation:
Handprint your name, address, day/evening phone number and email address (if available) on a 3”x5” card and mail in an individual envelope to: The Golden Ticket $100,000 Joint Fundraising Campaign Sweepstakes, 841-853 Ocean Parkway, Brooklyn, NY 11230. Please write one option from (i)-(vi) above on the front of your envelope to indicate the drawing(s) you wish to enter. For example, if you write (vi) on the front of your envelope, you will receive thirty (30) entries each in all five (5) drawings. If you forget to write an option on the front of your envelope, you will receive the maximum number of thirty (30) entries in all five drawings. Your mailed entry must be received by 7/17/2013. Limit: Thirty (30) entries per person in each of all five (5) drawings regardless of the method of entry.
2. ELIGIBILITY
The Sweepstakes is open to legal residents of the 50 United States and the District of Columbia, residents of Canada (except void in Quebec), and other countries only where specifically offered and permitted by law, who have reached the age of majority in the jurisdiction in which they reside. Sweepstakes is void in the province of Quebec and where prohibited and restricted.
3. CONSUMER DISCLOSURES
You have not yet won. Ends 7/10/13. No purchase necessary. Approximate odds of winning and prize retail values (“ARV”): One (1) First Prize: $100,000.00 cash. (ARV: $100.00.00): 1:49,000. One (1) Second Prize: $10,000.00 cash or, trip (ARV: $10,000.00): 1:49,999. One (1) Third Prize: $5,000 cash or trip (ARV: $5,000.00) 1:49,999. Five (5) Fourth Prizes: $1,000.00 (ARV: $1,000.00 each)1:10,000. Ten (10) Fifth Prizes: $500 Gift Card for merchandise at a national online retailer (ARV: $500.00 each): 1:5,000. Total approximate retail value of all prizes to be awarded: $125,000.00 maximum value. Cash prizes will be awarded in the form of a corporate check. All prize values are in US currency. Except as set forth above, prizes may not be substituted or transferred, except by Sponsor in its sole discretion. Sponsor reserves the right to substitute a prize of equal or greater value. Gift Cards will not be replaced if lost or stolen once it has been awarded. First through Fourth Prize winners (if a US resident) will receive a 1099 reflecting the value of their respective prize(s) as required by law. Sponsor will withhold 30% of winnings from any winner outside of the United States according to U.S. Internal Revenue Service requirements. U.S. residents are responsible for paying all applicable taxes.
4. SELECTION OF WINNERS
Winners will be selected on or about July 24, 2013 in a random drawing from among all eligible entries received for a given drawing. The odds of winning in any Drawing depend on the number of eligible entries received as stated in Rule 3. Decisions of the judges are final. Winners will be notified by email and/or US mail, in the sole discretion of the Sponsor. If a potential winner fails to respond after two notices, his/her prize may be forfeited and an alternate winner may be selected.
If offered in Canada, Canadian winners will be required to correctly answer a mathematical skill testing question administered via phone as a condition of receiving a prize.
5. CONDITIONS OF PARTICIPATION
Grand and First Prize winners may be required to sign and return an Affidavit/Declaration of Eligibility and a Liability and Publicity Release (where legal) within fourteen (14) days of notification. Non-compliance with these requirements may result in disqualification and an alternate winner selected at random. In the event that a prize or prize notification is returned as undeliverable, such prize will be forfeited and an alternate winner selected at random at Sponsor’s discretion.
By participating, entrants release and agree to hold Sponsor and Participating Promoters harmless from any liability for any injuries, losses or damages of any kind resulting from acceptance, possession or use/misuse of any prize or participation in the Sweepstakes. No responsibility assumed for lost, late, misdirected, mutilated, damaged, incomplete, illegible or postage due/entries. All such entries will be disqualified. All entries become the property of their respective Participating Promoter and will not be returned or acknowledged.
Except where prohibited by law, each winner agrees that Sponsor and Participating Promoters may use the winner’s name, address (city, state/province/country, photograph, videotape, likeness, and/or prize information for advertising, publicity and promotional purposes without further notice and/or compensation.
6. ERRORS
Sponsor and Participating Promoters are not responsible for electronic, technical or mechanical malfunctions of any sort, for any late, lost or mishandled entries, or for any typographical errors in the advertising of the Sweepstakes. Sponsor reserves the right, at its sole discretion, to cancel/suspend/modify the Sweepstakes if it becomes corrupted in any manner such that it cannot be conducted as originally planned and, in such event, to select winners by random drawing from all eligible entries received prior to the action taken by Sponsor.
LANGUAGE DISCREPANCY
In the event there is a discrepancy or inconsistency between the English version and any other language version of these full Official Rules or of any promotional materials, the English version will govern and control.
7. DISPUTES/CHOICE OF LAW
Each entrant agrees that this Sweepstakes shall be governed by the laws of the State of New York and: (1) any and all disputes, claims and causes of action arising out of or connected with this Sweepstakes or any prize awarded shall be resolved individually, without resort to any form of class action, and exclusively by state or federal courts situated in New York (2) any and all claims, judgments and awards shall be limited to actual out-of-pocket costs incurred, but in no event attorneys' fees; and (3) no punitive, incidental, special, consequential or other damages, may be awarded (collectively, "Special Damages"), and (4) entrant hereby waives all rights to claim Special Damages and all rights to have such damages multiplied or increased.
8. WINNERS LIST
For the names of prize winners send a self-addressed, stamped envelope to United Lubavitcher Yeshiva of Ocean Parkway, 841-853 Ocean Parkway, Brooklyn, NY, 11120. List will be available beginning on or about August 30, 2013.
